Preparing for your appointment
For your first visit, plan to arrive at least 20 minutes before your scheduled appointment to allow us to obtain the information needed to register you. Bring the following information with you:

  • personal medical history
  • family history of medical problems
  • complete list of doses and kinds of medications that you are currently taking, including:
  • over-the-counter drugs
    vitamins
    supplements
    herbal medications
complete list of any allergies to medications

  • name and telephone number of your pharmacy
  • name of referring physician and primary care provider (if applicable)
  • your insurance identification card and any copayment fee that your carrier may require
  • paperwork required to process your insurance claim

If you have undergone prior testing for any eye disease, you may be asked to bring a copy of your test reports. Our specialists will review the results to come up with the most comprehensive approach to your problem.

Receiving test results and prescriptions

  • test results
Your doctor will not contact you if your test results are normal; however, if you wish to know the results, you may contact us at 412-648-6161, and a member of our staff will assist you. In addition, in the event of an abnormal test result, you will be contacted by your doctor.
  • prescriptions
When you see your doctor, ask for enough prescription refills to last until your next appointment. If you find that you do not have enough refills, call during regular office hours to request your refill. We will need to know the name of your medication, the dose, how often you take it, the name of your doctor, your health insurance plan, and your pharmacy’s telephone number. Please do not wait until you run out of medication to call the office.

Insurance or Payment Questions
We accept most major medical insurances. All copays will be collected on the day of service. For any services provided that are not covered by your insurer (such as contact lenses), payment is expected at the time of service. Call us if you have questions about the health insurance we accept or about our payment policies.
